Majority of Americans are morons and don't know how it works, since minors don't get gender surgeries often enough to be an issue.
It's not that it never happens, but it sure as hell isn't done willy nilly, and only with and after a wide range of consultation and planning. Hell, the last guidelines I saw were still focused on puberty suppression medications until the late teens anyway. The recommended ages from international trans medicine groups for hormones is still in the early teens, with surgeries not only being held until the late teens, but still needing parental consent the way any other form of medical treatment is.
