I am not filling out the form.

The first thing that hit me is that these pages are formatted in a way that is unpleasant to read; it feels it's a chore.

I would recommend making the pages 2 columns instead of 1. It's a quick change that really makes a difference without needing to change the wording. The effort to payoff ratio is on par with learning to drybrush or ink a miniature.
