This is one I made, but I can't say it's fully custom, as I edited Jasmine's front sprite for this character's front sprite, and got lazy with the trainer card sprite and just copied the front sprite.
Tetris & Dr. Mario does not save high scores. No battery save, so when you power off the console, high scores are wiped.
But there's a hack you can apply (to the PAL rom) to make it save high scores!
