Must be fake (unless you're driving a lens-less camera obscura). There's a region starting from the camera surface until its focal point at which the camera cannot focus objects. This is why macro lenses that have a very small focal length exist. And the insect looks very clear despite being closer than the camera could focus. Also, in a real photo, the mosquito would not look transparent like that. It would look pitch-black like everything else in the photo. The background seems legit to me. It can be produced if you set narrow aperture, short exposure and low light-sensitivity, knowing this was made in a foggy day. But that mosquito is an overlay, IMHO.

To add on, not just that but also somehow the shutter speed is fast enough to perfectly capture the mosquito with no blurring, but simultaneously slow enough to cause some minor blurring on the pedestrians. Additionally, the mosquito is flying but its wings are at rest.

More fun policeage: An insect of that size would suffocate pretty quickly. They don't have a lung and circulatory system, but rather just have holes in their body through which air can travel deep inside and then the oxygen kind of just permeates to everywhere it's needed.

If the insect is scaled up (and oxygen concentration not scaled appropriately), then the oxygen will simply not permeate deep enough.
