Nah, that would be manageable.
Scenario 1: everybody gets the same ads -- use the same technique used for sponsor block. The first people to see the video mark the ads for everyone else, the people who watch the videos later just skip the ads.
Scenario 2: people get different ads. Viewer 1 and viewer 2 watch the same video, compare their streams and any time their streams differ, that's an ad. Jump forward until you're past the section where the two streams differ.
