Is the pre amp powered over the coax? The filter may not be designed to handle power.
Nearby lightning could have fried the filter. A direct strike is not needed to cause damage.
It could just be terrible build quality. The filter could have had a bad solder joint that cracked.
Dam, my old antenna was powered over the coax. That's probly what did it.
